I, the undersigned participant, hereby acknowledge and agree to the following terms and conditions in consideration for being permitted to participate in the yoga class provided by Pawsitive Stretch Michigan LLC:
1. Assumption of Risk:
I understand that participating in a yoga class involves interactions with animals and physical activities associated
with yoga. I am aware that there are inherent risks involved, including but not limited to the possibility of scratches,
bites, or other injuries resulting from interactions with animals, as well as the risk of physical strain or injury from
yoga movements. I voluntarily assume all such risks associated with the minor’s participation.

2. Physical Condition and Medical Advice:
I affirm that the minor participant is in good physical condition and has no medical conditions that could prevent or
hinder their safe participation in the yoga class. I affirm that I have consulted a physician regarding any concerns
about the minor’s ability to participate, especially if the minor has any allergies, phobias, medical conditions, or
injuries that could be affected by interactions with animals.
3. Animal Handling:
I understand that while the minor participant is participating in the class, they will have the opportunity to interact
with animals. I agree to ensure that the minor handles the animals gently and responsibly, and we will follow any
instructions provided by the class instructor regarding proper animal interaction.
4. Release and Waiver:
I hereby release and forever discharge Pawsitive Stretch Michigan LLC, its instructors, employees, volunteers, and agents from any and all claims, demands, causes of action, damages, or liabilities, whether arising from negligence or otherwise, resulting from or in any way connected with the minor's participation in the yoga class, including interactions with animals.
5. Indemnification:
I agree to indemnify and hold harmless Pawsitive Stretch Michigan LLC, its instructors, employees, volunteers, and agents from any and all claims, demands, causes of action, damages, or liabilities, including attorney’s fees and costs, arising from the minor's participation in the yoga class or my breach of any terms of this agreement.
6. Media Release:
I grant Pawsitive Stretch Michigan LLC the right to use photographs, videos, or other media of the minor taken during the yoga class for promotional or educational purposes without compensation.
7. Liability for Lost or Stolen Property
I understand that neither Pawsitive Stretch Michigan LLC nor any Venue host locations provide secure storage for personal belongings. Pawsitive Stretch Michigan LLC shall not be responsible or liable for any loss, theft, or damage of any personal property belonging to the Participants while left unattended.

The Liability Waiver Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I HAVE CAREFULLY READ THIS LIABILITY WAIVER AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT BY SIGNING THIS WAIVER, I AM WAIVING CERTAIN LEGAL RIGHTS I, MINOR PARTICIPANT, OR OUR HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, AND ASSIGNS MAY HAVE AGAINST Pawsitive Stretch Michigan LLC.
